A South Wales health board is investigating how mistakes were made which led to a grieving family cremating the wrong person.

Bosses at The Grange Hospital in Cwmbran say 'words can't express' how sorry they are after the mix-up led to a stranger's body being cremated while the family paid their respects.

An initial review found it was down to human error and an isolated case but investigations by Aneurin Bevan University Health Board are continuing.

The family was forced to have a second funeral after the mistake came to light.

The health board has apologised and assured the family they will be kept up to date throughout the investigation.
