Former Nottingham Forest and Everton midfielder Gemmill named his first squad on Tuesday after replacing Ricky Sbragia as Scotland's youth team chief and included 10 new faces.
But Cadden, who went off with a hamstring problem in Motherwell's 3-1 defeat by Hearts on Friday, and on-loan Raith Rovers defender Smith have pulled out.
Rangers midfielder Liam Burt and Derby defender Calum MacDonald have been called up in their places.
Scotland face Iceland in Reykjavik on Wednesday before a trip to Macedonia on October 11 in European Championships qualifiers.
The Scots are second bottom of their six-team group and out of the race to reach next year's finals in Poland.
