Suffolk man was wrongly invited for Covid-19 jab before dying from a blood clot on his brain
A report out today has found there were delays in getting Jack Last the right treatment
A report has found a from Suffolk who died from a blood clot on his brain was wrongly invited for a Covid-19 jab early.
27-year-old Jack Last died in hospital on the morning of the 20th April 2021, which a coroner concluded was a direct result of a blood clot, caused after he was wrongfully administered the AstraZeneca vaccine.
On March 20th 2021, Jack received an invitation from the local NHS booking system to arrange a Covid-19 vaccination, after 'system errors' identified Jack as living with parents, which gave him household contact status.
The came about due to errors in how the Suffolk GP Federation searched for household contacts of eligible patients.
One of Jack's parents had a previous illness that was not active at the time, but the illness code on their GP record led them to being classified as being eligible for vaccination.
Jack booked an appointment, and went for the jab on the 25th March.
During the evening of the 6th April, Jack started to feel unwell with a severe headache and disturbed vision.
Following a call with NHS 111, Jack went to West Suffolk Hospital and later that day admitted to the hospital for various tests.
The 27-year-old remained there until the 11th April when he started significantly deteriorating and was transferred to Addenbrookes Hospital in Cambridge.
Jack remained at Addenbrooke's Hospital until he died on the 20th April.
An inquest into his death in December 2022, the coroner recorded a narrative conclusion: that Jack died of a blood clot to the brain caused directly by the administration of the Oxford AstraZeneca Covid-19 vaccine.
In May 2021, healthcare consultancy Facere Mellius were formally commissioned to undertake an independent investigation to review the vents that led to a Jack being invited for a vaccination, and the care and treatment provided to him from March 2021 until the date of his death in the following April.
In a statement from Jack's family, they say Jack was a happy, healthy, carefree 27 year old with a kind smile.
It goes on to say: "Before Jack had his Oxford Astrazeneca COVID-19 injection, he hadn’t had a single day sick from work. He was very rarely unwell. Then suddenly he was not fine. It all happened so quickly it struggles to hit home at times that we are never going ot see him again."
"Conversely, the time from him being invited for his Oxford Astrazeneca COVID-19 injection, to dying, somehow also seemed to last forever, waiting to hear that actually, he would be OK. But of course he was not OK and none of this will ever be OK.
