Derby and Burton NHS Trust's apologised for failings which led to the death of a mum from Newark.

Jodie McCann died after suffering a series of cardiac arrests.

An inquest into the 22 year old's death heard she'd been fit and well before suffering gallstone pancreatitis last year.

The student nurse was originally treated in Sutton-in-Ashfield - before being transferred to Burton-on-Trent hospital.

Jodie was found unresponsive and doctors found she'd suffered organ failure.

In a statement, University Hospitals of Derby and Burton NHS Foundation Trust said she deserved a far better standard of care and it was truly sorry for the mistakes made.

Officials have told us a clear process and procedure has been put place for daily airway trolley checks to be completed and signed for, with a clear audit plan

The trust's death review process has also been enhanced to include weekly meetings overseen by an executive director to ensure timely, appropriate management of and learning from incidents.

Enhanced training sessions have been run for department staff, with additional airway study and simulation days booked to support additional training.

Dr Sreeman Andole, Interim Executive Medical Director at UHDB said:

"Jodie needed and deserved a far better standard of care than was provided to her and we are truly sorry for these failings.

"Our commitment to Jodie's family and our community is that we will build on the changes we have already made, and learn from Jodie's tragic case.

"We accept the Coroner's findings in full and we have a clear action plan in place to continue to address the concerns and make the further improvements needed as a priority."
