A coroner says she can’t be sure that a Malton nurse wanted to take her own life - after going into the river Derwent.
Victoria Taylor went missing last September - and her body was recovered from the water three weeks later.
Ms Taylor, 34, disappeared from her home in Malton, North Yorkshire, on September 30. She was originally from Huddersfield.
The inquest heard how Victoria known as 'Vixx' had a history of poor mental health, and had on a previous occasion been rescued after going into the river.
The coroner said her 'erratic behaviour that morning suggests she may have been having hallucinations' and therefore she 'can't be sure she intentionally took her own life'.
Members of her family cried during the evidence - with her eldest sister Emma speaking briefly, accusing NHS leaders of failing Vixx, by not offering the support she needed.
Giving evidence, senior members of staffs at Tees, Esk & Wear Trust say the care plan for Victoria 'felt appropriate to them at the time' but a 'learning review is being carried out'.
