People in Bridlington are being offered support on a range of issues this Winter.
The Humber Teaching NHS Foundation Trust’s Your Health Prevention and Lifestyle Services team is based within your local community and GP surgeries.
It's open and free to use for residents who need support with their health and wellbeing.
The Your Health Social Prescribing team in particular is keen to encourage residents to seek that additional support and defines its service as ‘meeting the needs of an individual or community by working together to improve their health and wellbeing needs’.
Offering a series of one-to-one motivation and advice-based sessions, the team is on hand, to help you feel empowered to improve your lifestyle.
In particular, the Social Prescribing team specialises in:
• Welfare and Financial health
• Housing
• Healthier lifestyle
• Emotional wellbeing
• Education
• Social Health
• Volunteering
• Lessen Isolation and loneliness
Service Manager at Humber Teaching NHS Foundation Trust, Natalie Belt, said:
“Social Prescribing is a great way to empower communities and individuals, to help them utilise skills and assets they already have, whilst also equipping them with new tips and support to improve their health and wellbeing.
Each individual we work with gets a personalised improvement plan which aims to motivate and support them through their journey to better health.”
