Who is weight loss surgery recommended for?

At BMI (body mass index) levels above 35, you are carrying around 50% more weight than is healthy for your height. This has implications for deterioration in health and long-term risks of developing conditions such as diabetes and high blood pressure.

At this point the National Institute for Health and Care Excellence (NICE) recommends surgery can be considered if patients have tried non-surgical measures but have failed to achieve sustained weight loss. It is important to meet with your GP to exclude a medical or psychological contribution to weight gain.

I want to lose weight but i'm not sure surgery is right for me.

Weight reduction without surgery is the safest approach but most patients I meet have had disappointing results with all other methods. However a whole lifestyle approach to diet and exercise is vitally important even after weight loss surgery to prevent weight regain.

Losing weight requires a calorie “deficit” (reducing calories in your diet). Although the recommended daily intake for women is less than 2000 calories, weight loss requires a daily reduction by around 600 calories. Surgery has the benefit of achieving guaranteed weight loss through permanent calorie reduction.

Which is the best procedure for me?

This is a question to be discussed with your surgeon. Operations are either “restrictive” reducing the amount of food you can eat (gastric band or sleeve gastrectomy) or are a combined “restriction and reduced calorie absorption” operation such as the gastric bypass. Weight loss operations are usually performed using keyhole surgery under a general anaesthetic.

The gastric band requires the placement of an inflatable ring around the top of the stomach connected to an access port buried under the skin that requires injections of fluid to achieve a balance of tightness that achieves fullness without side effects.

The sleeve gastrectomy permanently removes 80% of the stomach leaving a narrow tube creating a permanent state of feeling fuller more quickly without need for any adjustment.

The gastric bypass divides the stomach (without removal) attaching it directly to the small bowel; downstream your surgeon rejoins the part of the small bowel where the digestive enzymes enter so you effectively digest and absorb less of your food.

You can expect to lose around half of your excess weight with a gastric band and more than two-thirds of your excess weight with the gastric bypass. The sleeve gastrectomy has been shown to achieve weight loss results almost as good as the gastric bypass. Most weight loss is achieved within the first year where it stabilises.

How will an operation affect me?

All operations make you feel full after a much smaller portion size (the size of a tea cup). Essential daily vitamin supplements are required long-term to avoid nutrition complications. A dietitian is a vital member of the team caring for you before and after your operation.

Weight loss surgery can have dramatic improvement in fertility. Contraception is an important consideration even if you are planning to start a family as pregnancy is not recommended within the first 18 months to ensure the baby develops healthily.

Are there ways to help me lose weight without an operationN?

The gastric balloon is a temporary device used to lose weight that patients with a BMI of 27 or above can consider. This procedure involves placement of a balloon that is inflated inside the stomach during a 15 minute telescopic (endoscopy) procedure. It does not require a general anaesthetic and many patients are able to go home the same day. During the 6 months that the balloon is inside the stomach patients find they can lose as much as one-third or more of their excess weight.

How safe is the surgery?

With many years of experience in this surgery, serious complications are now rare and the most recent UK data shows less than 3 in 100 patients have complications related to their surgery and the risk to life is less than 1 in 1000. It is now safer than many other common operations.
