The boy was walking near Dedridge Pond in Livingston, West Lothian, at around 5pm on Friday when he was approached by the pair and knocked to the ground.
He suffered a facial injury but managed to make his way home with the help of an unidentified man, and was later treated at St John's Hospital.
Officers are trying to trace the two suspects, as well as the man who helped the 15-year-old in case he has more information.
The suspects are both white, aged in their late teens, with medium builds and around 5ft 10in.
Detective Constable Craig Waddell said: This was a frightening and upsetting attack on a young man and we are eager to trace the two males responsible as soon as possible.
We would also call on the man who came to the aid of the 15-year-old to come forward to help us progress our inquiries.
Anyone who was in the area at the time, or who recognises the description of the suspects, is asked to come forward immediately.''
